This study aims to evaluate the effectiveness of holistic midwifery care in reducing anxiety levels among third-trimester pregnant women. This study aims to evaluate the effectiveness of holistic midwifery care in reducing anxiety levels among third-trimester pregnant women. This research used a pre-experimental design with a one-group pre-test and post-test approach. A total of 30 third-trimester pregnant women visiting antenatal care services at City Public Health Center (Puskesmas Kota) were selected using purposive sampling. The intervention was conducted over two weeks in four sessions, which included pregnancy education, relaxation techniques, social support (involving husbands), and spiritual enhancement. Anxiety levels were measured using the Hamilton Anxiety Rating Scale (HARS) before and after the intervention. Data were analyzed using the Wilcoxon Signed Rank Test. The results showed a significant reduction in maternal anxiety scores following the intervention. The mean anxiety score decreased from 27.07 ± 4.66 before the intervention to 19.30 ± 4.90 after the intervention, with the Wilcoxon Signed-Rank Test indicating a statistically significant difference (Z = −3.873, p < 0.001). The intervention demonstrated a large effect size (r = 0.71), suggesting a substantial improvement in maternal psychological well-being. These findings suggest that holistic midwifery care may contribute to reducing anxiety among third-trimester pregnant women. However, given the one-group pretest–posttest design without a comparison group, the findings should be interpreted with caution. Integrating holistic approaches into routine antenatal care may support maternal mental health, although further controlled studies are needed to confirm these findings.
